Free JSA Template

What's on it
- Job details — the task, location, company, date, who prepared it, the supervisor, permit numbers and the emergency contact. Thin, deliberately: a header that takes half a page steals room from the analysis.
- The step table — Task, Hazards, Risk before, Controls, How it is implemented, Risk after. One row per hazard, not per step: two hazards in one step need two ratings, and a single band would hide one of them.
- The risk matrix — probability down the side, severity across the top, four bands. The same matrix the generator prints, so a document written by hand on this form and one produced by the app can be read side by side.
- Crew sign-off — ten rows of name, company, signature and date. An analysis the crew never saw proves nothing.
How to fill it in
Work left to right across one row at a time, and write controls strongest first: eliminate the hazard if the job allows it, substitute something less hazardous, engineer it away, then administrative controls, and PPE last for whatever remains. Score the hazard before controls, then again as if the controls in that row are genuinely in place — if the band doesn't move, the controls aren't doing anything. When a paper template stops being enough
Printed forms are fine for a one-off. They start costing you when the same job comes round every week and somebody retypes it; when a reviewer wants last month's version and it's in a truck; when the crew signs a sheet that then lives in a folder nobody can find after an incident; or when the hazards get copied forward from a job that wasn't quite this one.
That's the point at which generating it pays for itself: the draft arrives written for your actual activity, the controls come out in hierarchy order, the risk bands are scored for you, and the crew signs by scanning a QR code on the PDF — the signatures land on the document's own record with each signer's name and the time on their phone.
